St. Paul's PACE San Diego in downtown San Diego's Bankers Hill district opened in 2008 at 111 Elm Street as the first Program of All-Inclusive Care for the Elderly in San Diego County, operated under the St. Paul's Senior Services umbrella that has served the local senior population since 1960. The flagship Desdemona Reasner Wellness Center provides primary medical care, geriatric-specialist physicians, dental services, physical and occupational therapy, prescription management, and hot three-course meals under one roof for enrolled participants, coordinating overlapping social support with organizations like Senior Community Centers serving the same population. PACE covers all medically necessary services through Medicare and Medi-Cal funding—including 24-hour emergency medical support, in-home care, and round-trip transportation—eliminating the fragmented navigation that typically burdens seniors with chronic conditions. The program has served more than 4,000 San Diego seniors across four clinic locations, with wellness center activities ranging from exercise classes and art workshops to live musical entertainment, building a social infrastructure that reduces isolation alongside food-access organizations like the Alpha Project - Food Distribution Center serving the downtown corridor. The PACE model's most complex care capacity involves managing concurrent chronic conditions—dementia, diabetes, cardiac disease, and mobility impairment—through a single interdisciplinary team that replaces the need for assisted living or skilled nursing facility placement.
