Operated by San Diego nonprofit St. Paul's Senior Services — established in 1960 as the first HUD senior-housing building in California — St. Paul's Skilled Nursing and Rehabilitation runs a boutique 59-bed skilled nursing community on Nutmeg Street in Bankers Hill, remodeled in 2018 with private and semi-private rooms, individual climate control, and in-room cable. Short-term rehabilitation for post-surgical and post-illness recovery pairs continuous licensed nursing coverage with in-house physical, speech, and occupational therapy, with outpatient hand-therapy continuation routing to Advanced Hand Therapy for residents returning home after upper-extremity procedures. Both short-term rehab and long-term care admissions are accepted, with each resident accessing the same amenities, activities, and spiritual care programming regardless of length of stay. Hospice integration operates through the facility's continuous licensed nursing model combined with outside hospice agency partnerships, allowing residents end-of-life care without a building transfer. Post-discharge continuity for medically complex residents who still need specialist primary care routes through Scripps Advanced Care Clinic for complex-care transitions after rehab. Medicare and Medi-Cal are both accepted, with an allotted number of Medi-Cal beds alongside private-pay options for residents whose post-acute recovery crosses coverage boundaries. The most involved rehabilitation cases pair post-stroke or post-orthopedic recovery with chronic-disease management and ADL retraining inside an individualized care plan built around each resident's return-to-home goals.
