St. Paul's PACE Chula Vista has provided all-inclusive geriatric managed care on L Street since 2012, operating as the PACE Akaloa center within St. Paul's Senior Services — a San Diego nonprofit founded in 1960. The center delivers primary medical care, medication management, physical therapy, and specialty referrals under a single capitated Medicare/Medi-Cal plan, coordinating lab work and diagnostics through facilities such as Chula Vista Family Health Center. An interdisciplinary team of geriatricians, nurses, and social workers builds each participant's individualized care plan, with door-to-door medical transportation eliminating one of the most common barriers to senior healthcare access. On-site rehabilitation includes physical and occupational therapy sessions designed to maintain mobility and independence, with participants requiring advanced musculoskeletal intervention referred to specialized outpatient providers including Manual Orthopedic Physical Therapy for complex orthopedic cases.
